1. Identification details of the declarant, hereinafter referred to as "the Declarant"<\/h2><\/div><\/div><\/div>

The declaration can only be sent by the person authorised to represent the prospective Partner of the e-Chamber. If the declaration is not completed by:<\/p>\n

    \n
  1. a person authorised to represent the Declarant in accordance with the National Court Register, or<\/li>\n
  2. a sole trader themselves,<\/li>\n<\/ol>\n

    please prepare a power of attorney signed by the persons authorised to represent the Declarant. A scan of the signed power of attorney should be emailed to\u00a0biuro@eizba.pl<\/a>. A specimen power of attorney can be found below in the declaration.<\/p>\n

    In the case of the so-called multi-person representation of the company<\/strong>, when the declaration is completed by one of the authorised persons, please attach a scan of a power of attorney signed in accordance with the company’s representation rules, from which the authority to submit the declaration independently arises. The power of attorney should be emailed to\u00a0biuro@eizba.pl<\/a>.<\/p><\/div><\/div><\/div>
